Gord, 

I am wondering if there is any other way that excessive water can get into the exhaust. What about the exhaust manifold heat exchanger?  I wonder if it would be worth while to do a differential compression test and see or hear if there might be leaks via the water jacket prior to removing the head? What about removing the spark plugs to see what they look like? Use a flashlight to peer into the cylinders. Water will show up as beads separated from the oil. Then pull the HT lead from the coil and turn the engine over with the starter and have someone observe what if anything comes out of the spark plug holes.  

Contact the A-4 guru at Van Ness and ask about the possible causes and their probability. Do a bit more head scratching prior to exploratory surgery, my two cents.
