Suggest you read Don Casey's This Old Boat - he has complete instructions on how to make them.  They are quite easy to make, you need a saw, a hole cutter and something to smooth the edges - a router or just a belt sander.  They are made in pairs to match the inside and outside exactly.

Hi John,

I have my inside and outside  hand rails off the boat for an inside
paint job and deck refinishing.
The inside handrails are exactly the same size of the outside handrails.
The fasteners go to and from each handrail.
I could make a couple of sketches/drawings, maybe even a template. I can
take pictures as well if you wish and email them to you.
