My feeling on this subject is that if you have a hard mounted engine like an 
Atomic 4 then your alignment should be damn close to perfect. If you are 
mounted on flexible mounts, as so many diesels are, then the alignment will 
change between static (neutral) and when there is a load on the engine. 
Since we do not have thrust bearings in our boats, when the engine is 
engaged, in either forward or reverse, the engine moves (slightly) and the 
angle between the engine and drive shaft changes as a result. Therefore I 
believe that it is essential to have a flex-coupling at the shaft to make up 
for the minor angle change. I am no engineer - but this just makes sense to 
me. When I changed my engine 6 years ago (all work done by myself) I bought 
the best flex-coupling I could find and have had smooth operation at all RPM 
levels since.

I am by no means the house guru on diesel alignments (I've paid for a
few--does that count??) but I would say that if you have anything like
significant additional vibration when the engine is in gear (compared to in
neutral) then you may have a problem that bears looking into.  I had a
problem with a rear engine mount last year that cost me a new shaft,
coupling, etc, because, essentially, I didn't know how much vibration was
too much, and I didn't have it checked out. A stitch in time.....
