Just to throw a little more gas on the fire.  On Isa Lei I normally use a nylon web ladder to go up.  It is one that attaches to the track.  However I also use a bos'ns chair.  The ladder lets me use my legs to climb, while my wife keeps the chair tight against my butt.  This lets me do the work of going up, with the safety of a chair.  She keeps turns on the winch and keeps the slack out of the halyard.  If I am forced to go up alone I use a safety line with a turn around the mast and keep sliding it as I go.  While the ladder scares hell out of me, it lets me use my legs and I stay close to the mast.
